USEFUL FORMS

Parents and Guardians are required to provide consent for all activities that cadets undertake. Once we have the initial consent to join ('3822A - Consent to Join' and '3822H Health Declaration'), they are legally able to enrol into the organisation and we will require no further consent for them to take part in Squadron HQ based activities.

 

 Activities 'off Squadron' - such as flying, camps, sports competitions etc, will require a 'TG21 Activity Consent Form' and a 'TG23 Health Declaration' as appropriate. We currently require a signed, paper version of these for every activity.

If a cadet has been selected to go flying we will also need a 'Av Med Form 1'. NOTE: If a cadet has any of the medical conditions listed they may need to see their GP for them to certify the cadet as fit to fly. If this is the case please contact the Sqn immediately as we will need to issue you with an additional form to present to the GP.
